Disclaimer and Waiver

As a client of Our Eudaemonia with Gurvinder Dhol, I will participate in modalities from Energy Psychology (EP), such as the Emotional Freedom Technique (EFT or Tapping), Quantum Emotional Freedom Technique (QEFT), also known as Intergenerational or Ancestral Work, as well as Qigong and Meditation.

I acknowledge that Tapping incorporates principles from acupuncture, cognitive reframing, exposure therapy, and other established healing practices. It is a component of a recently emerged set of energy-based coaching and self-development techniques known as Energy Psychology. By tapping specific acupoints on my hands, head, face, and upper body while addressing emotional issues, it may assist in reducing the physiological “stress response” and returning the body to a state of calmness.

I fully agree to assume and accept full responsibility for any and all risks that may be associated with Tapping. I further understand and agree that the information presented in my session is for my personal use only and not for application to others.

I understand that Tapping may lead to the emergence of unpleasant emotional or unresolved memories, as well as emotional distress and physical sensations in my body related to my life experiences. Emotions may continue to surface after the session. Our Eudaemonia, Gurvinder Dhol, will equip me with tools and techniques to self-support until our subsequent session. Additionally, I acknowledge that I can discuss these emotions with a qualified healthcare provider, such as a licensed Psychologist, Psychiatrist, Social Worker, or Mental Health Professional/Counselor.

I acknowledge that the techniques mentioned above are not intended to diagnose, treat, cure, or prevent any physical or psychological ailments, illnesses, or mental health disorders. Our Eudaemonia, Gurvinder Dhol, is not a licensed Psychiatrist, Psychologist, Social Worker, or Mental Health Professional/Counselor, and will not provide diagnoses for mental illness or prescribe medication. I understand that I will not receive mental health counseling as part of these techniques.

Assumption of Risk and Release of Claims

I understand that Our Eudaemonia, Gurvinder Dhol, may be legally compelled to testify in a legal proceeding in which I am involved, potentially requiring disclosure of information shared during my session. Despite Our Eudaemonia and Gurvinder Dhol’s commitment to maintaining confidentiality to the fullest extent allowed by law, I acknowledge the inherent risk that she may be subpoenaed and obligated to reveal personal information about me that would otherwise remain confidential.

Information shared during sessions with Our Eudaemonia, Gurvinder Dhol, remains confidential. However, in cases involving minors (aged 19 and under) where their safety is a concern, Our Eudaemonia, Gurvinder Dhol, is legally obligated to report to the relevant authorities. Additionally, she may consult with her professional mentors for development purposes.

In addition, I understand that by using Tapping, it is possible that previously vivid or traumatic memories may fade. This could adversely impact my ability to provide detailed legal testimony regarding a traumatic incident.

The information shared during the coaching sessions, encompassing EP, Tapping, QEFT, Qigong, and Meditation, is aimed at personal development. It is important to note that these techniques are not designed to diagnose, treat, cure, or prevent any disease, illness, addiction, or other psychological, mood, impulse control, emotional, behavioural or mental health disorder. They should not be considered a substitute for professional medical, psychological, psychiatric, or other mental health treatments.

The testimonials, stories, and information shared with me should not be viewed as a guarantee, warranty, or prediction of the outcomes I may experience from using EP, Tapping, QEFT, Qigong, and Meditation for specific purposes. Our Eudaemonia, Gurvinder Dhol, does not offer any warranty, guarantee, or prediction regarding the results I may achieve. Testimonials are presented as they were given, with minor adjustments for clarity or grammar, and individuals providing testimonials have not received any material compensation. While Tapping has shown remarkable clinical results and is supported by scientific evidence, it is essential to recognize that it is not a replacement for psychiatric, psychological, or medical treatment. Furthermore, I acknowledge that there are no guaranteed outcomes for any individual or individual sessions.

Minors, 19 Years of Age and Under

For individuals under the age of 19, I understand that the law may grant my parents, or legal guardians the right to access my records. Our Eudaemonia, Gurvinder Dhol, follows a policy requiring a written agreement from parents or legal guardians, to waive their right to access my records. If they agree, Our Eudaemonia, Gurvinder Dhol, will only provide them with general information, unless there is a high risk of serious harm to myself or others. In such cases, Our Eudaemonia, Gurvinder Dhol, will notify them of her concern and provide them with a general summary at the conclusion of our sessions. However, before disclosing any information, Our Eudaemonia, Gurvinder Dhol, will first discuss the matter with me if possible and make efforts to address any objections I may have prior to discussing it with my parents, or legal guardians.

For Parents and Legal Guardians of a Minor (19 years of age and under)

I declare that I am the Parent or Legal Guardian of

I authorize my child or minor to participate in EP, Tapping, QEFT, Qigong, and Meditation sessions with Our Eudaemonia, Gurvinder Dhol. To foster a trusting relationship and maximize my child's or minors benefit, I waive my legal right to access their information. Nevertheless, I acknowledge that I can still receive general feedback under these circumstances.

Non-Traditional Complementary Healing Modalities

While EP, Tapping, QEFT, Qigong, and Meditation may offer benefits as non-traditional complementary healing modalities, it's advised to use them alongside treatment prescribed by my medical doctor and/or other qualified healthcare professionals. By continuing, I fully agree to release, indemnify, and hold harmless Our Eudaemonia, Gurvinder Dhol, her family, heirs, personal representatives, consultants, employees, and assigns from any claim or liability whatsoever for damage or injury, whether personal, financial, emotional, psychological, or otherwise, arising at any time during or in relation to my session(s). This includes information provided on the Our Eudaemonia website, social media, and any shared resources.

I acknowledge that I have carefully and completely read and fully understand all aspects of this Agreement and that I agree to all of the terms and conditions stated herein. This Agreement shall be binding upon me and my heirs and legal representatives.

Exclusive Jurisdiction

NOTICE: Our Eudaemonia, Gurvinder Dhol, operates under the laws of the Province of British Columbia, Canada, and this agreement falls within its jurisdiction. Any matters concerning EP, Tapping, QEFT, Qigong, and Meditation sessions are exclusively governed by the courts of British Columbia. The parties involved acknowledge that services provided by Our Eudaemonia, Gurvinder Dhol, will be conducted in/from the Province of British Columbia. The courts of British Columbia hold exclusive jurisdiction over any complaints, demands, claims, proceedings, or causes of action arising from these sessions or any other aspect of the relationship between Our Eudaemonia, Gurvinder Dhol, and Client.

Governing Law

The resolution of any and all disputes arising from or in connection with relationship, including disputes arising under or in connection with this agreement shall be governed by and construed in accordance with the Province of British Columbia, (other than the conflict of laws rules) and the Laws of Canada applicable therein.

If any court of law rules that any part of the Disclaimer is Invalid, the Disclaimer stands as if those parts were struck out. By continuing to work with Our Eudaemonia, Gurvinder Dhol, I agree to all the above. By completing the below, I acknowledge I am in full control of my intellectual abilities. The parties hereby agree that all aspects for the relationship between Our Eudaemonia, Gurvinder Dhol and Client.

Length of Sessions, Billing and Payments

Each session with Our Eudaemonia, Gurvinder Dhol, is scheduled for either 60 or 90 minutes, and I reserve the right to conclude the session at any time. By signing this agreement, I acknowledge my willingness to participate in sessions with Our Eudaemonia, Gurvinder Dhol, as outlined above. I understand that payment in advance is required at the time of booking.

Cancelled/Missed/Late Appointments

I acknowledge and agree to the following session policies:
1. Sessions will promptly end if I am late, without extending into the next client's session.
2. Failure to provide 24-hour notice for session cancellations will result in forfeiture.
3. Changes or cancellations must be made more than 24 hours before the session to avoid forfeiture.
4. If rescheduling is necessary, I will be flexible and aim to reschedule within 2 weeks. If unable to do so due to external circumstances, I may request a refund.
5. Refunds are available for cancellations made at least 24 hours prior to the session or in case of unforeseen circumstances preventing attendance.
6. If Our Eudaemonia, Gurvinder Dhol, is late, session time will be adjusted accordingly or added to a future session.
7. Sliding scale payment options are available for individuals facing financial hardship or for University/College students.

Contacting Our Eudaemonia, Gurvinder Dhol

If I am unable to reach Our Eudaemonia, Gurvinder Dhol, and cannot wait for her response via email, I will promptly contact my Family Physician/Nurse Practitioner. In the event of a medical emergency, I will immediately dial 911 or proceed to the nearest Hospital Emergency Room

Termination of Treatment

If Our Eudaemonia, Gurvinder Dhol, determines that she cannot provide the services I am requesting to a high professional standard for any reason, she will inform me of this decision and refer me to another practitioner who may better meet my needs. Should I verbally or physically threaten or harass Our Eudaemonia, Gurvinder Dhol, her office, or her family, she reserves the right to terminate my treatment immediately and unilaterally. Additionally, Our Eudaemonia, Gurvinder Dhol, reserves the right to refuse or terminate a session if I or anyone in the session is suspected of being under the influence of alcohol or mood-altering substances, and I will be responsible for the full payment of the fee.

Vacations

Our Eudaemonia, Gurvinder Dhol, may occasionally be absent from the office to spend time with her family or attend professional events. She will strive to provide advance notice of any extended absences and discuss alternative options for progressing towards my goals during her absence.

Complaints

If I am dissatisfied with the progress of our sessions, I will promptly communicate my concerns directly to Our Eudaemonia, Gurvinder Dhol. She will take my concerns seriously and respond with care and respect.
